| NEW HCPCS CODES PRESSURELITE® MOLDABLE
DIABETIC INSOLES |
| A new, permanent HCPCS codes has been established to replace the
temporary "K" code on our PressureLite® Moldable Diabetic Insoles. This new code becomes effective
January 1, 2006. Our suggested HCPCS codes will be changing as follows: |

There will be no grace period for using the old "K" codes after 1/1/06. HCPCS code descriptions will
remain the same and there will be no change to reimbursement amounts.
The PressureLite® Moldable Diabetic Insoles approved for the old, temporary "K" code should
automatically be reassigned to their new permanent "A" codes by SADMERC on 1/1/06.
